Position Summary
We are seeking a Full-Time Occupational Therapist (OT) to join our home care team. In this role, you will provide client-centered, in-home occupational therapy services, helping individuals improve their ability to perform daily activities, enhance independence, and improve overall quality of lifewithin the comfort and safety of their homes. Key Responsibilities
- Initial assessment/Evaluate clients functional abilities and therapy needs in the home setting, in accordance with physician orders.
- Develop, implement, and regularly update individualized treatment plans focused on ADLs, safety, and independence.
- Provide hands-on therapy and educate clients and families on adaptive techniques, energy conservation, and home safety.
- Recommend and train clients on adaptive equipment and assistive devices to support independence at home.
- Collaborate with interdisciplinary team members including RNs, PTs, caregivers, and physicians to ensure coordinated care.
- Maintain timely, accurate documentation of evaluations, progress notes, and discharge summaries in compliance with state and agency standards.
- Deliver compassionate, respectful, and culturally sensitive care to diverse client populations.
- Current, valid Massachusetts Occupational Therapist (OT) license .
- Valid drivers license and reliable transportation.
- Current CPR certification.
- Minimum 2 year of OT experience (home care experience is required).
- Strong communication, organization, and time-management skills.
