Role Summary

Our client, a healthcare provider delivering home health services, is seeking an experienced Registered Nurse (RN) to serve as a Patient Care Manager – Home Health Regional Floater . This role supports clinical operations across multiple locations, providing leadership, care coordination, and oversight of patient care plans.

The position requires a flexible nursing professional with strong clinical and leadership capabilities who can travel within the region to ensure consistent, high-quality home health services and regulatory compliance. The RN will collaborate with clinical teams, support staff development, and ensure patient care meets established healthcare standards.

Key Responsibilities
  • Oversee and coordinate patient care across multiple home health locations within the assigned region.

  • Conduct patient assessments and develop individualized care plans.

  • Ensure compliance with Medicare guidelines, home health regulations, and quality standards.

  • Provide clinical guidance and leadership to nursing staff and interdisciplinary care teams.

  • Support clinical staff in delivering safe, effective, and patient-centered care.

  • Foster strong relationships with patients and their families to support positive care outcomes.

  • Monitor documentation and clinical practices to ensure adherence to regulatory requirements.

  • Travel between regional locations as needed to support care management and clinical operations.

Required Qualifications
  • Active Registered Nurse (RN) license in the applicable state.

  • Home Health experience required , including proficiency with OASIS documentation .

  • Strong knowledge of Medicare regulations and home health compliance requirements .

  • Demonstrated ability to manage patient care and coordinate services across multiple locations.

  • Excellent communication, leadership, and interpersonal skills.

  • Ability to work collaboratively in a team-oriented healthcare environment.

Preferred Qualifications
  • 2+ years of supervisory or leadership experience in a home health setting.

  • Experience supporting clinical teams across multiple service locations.

Compensation & Work Model
