- Provide on-site training and coaching to individuals at community job sites to ensure job retention and skill development.
- Support individuals participating in community-based activities, including volunteer work and paid employment, that align with their interests and goals.
- Ensure timely transportation to and from job sites, volunteer shifts, and other scheduled activities, either by providing direct transportation or assisting with public/private transit.
- Deliver personal care and physical assistance as needed, adapting support techniques to meet individual medical and physical needs.
- Maintain accurate, timely, and complete documentation in accordance with agency and regulatory requirements.
- Promote and protect the legal, civic, and human rights of all individuals served, maintaining strict confidentiality at all times.
- Maintain a customer-focused approach in all interactions, ensuring quality service delivery and professionalism with individuals served, families, guardians, and referral sources.
- Collaborate with team members in planning, goal setting, and maintaining a positive and inclusive program environment.
- Actively participate in team meetings, training sessions, and program development activities.
- Assist in maintaining a clean, safe, and organized work environment by reporting issues to appropriate personnel.
- Complete all mandatory training to ensure compliance and professional development.
- Perform other duties as assigned by the supervisor and report any barriers to completing assigned tasks.
- High school diploma or GED equivalent.
- Experience in providing training, case management and appropriate service delivery to adults with intellectual and developmental disabilities living in the community.
- Candidates must be able to read, write, and comprehend English to accurately follow care plans, document services provided, and communicate effectively with individuals, families, and healthcare professionals.
- Basic computer literacy is required, including the ability to use email, word processing software, spreadsheets, and case management systems.
- CPR/First Aid; training is offered by Waystone to acquire certification within 3 months of employment.
- Valid Driver’s License in good standing with a minimum of one year of driving experience.
