We are looking for a dedicated and compassionate Chronic Registered Nurse (RN) to join our healthcare team in Peabody. The ideal candidate will possess a strong clinical background, excellent communication skills, and a passion for providing quality care to patients with chronic illnesses.

What You'll Do
Assess and monitor the health status of patients with chronic conditions
Develop individualized care plans in collaboration with multidisciplinary teams
Educate patients and families on disease management and self-care practices
Provide emotional support and guidance to patients throughout their treatment journey
Administer medications and treatments as prescribed by physicians
Maintain accurate patient records and documentation
What We're Looking For
Active RN license in the state of Massachusetts
Experience In Managing Chronic Illnesses Preferred
Strong interpersonal skills and empathy towards patients
Ability to work collaboratively within a healthcare team
Excellent organizational skills and attention to detail
