Note: The compensation range(s) in the table below represent the base salaries for all positions at a given grade across the health system. Typically, a new hire can expect a starting salary somewhere in the lower part of the range. Actual salaries may vary by position and will be determined based on the candidate's relevant experience. No employee will be paid below the minimum of the range. Pay ranges are listed as hourly for non-exempt employees and based on assumed full time commitment for exempt employees.

Medical Social Worker at Community Health Centers
Through Integrated Behavioral Health, our Community Health Centers are seeking a Medical Social Worker to join our team.

QUALIFICATIONS:
Bachelors of Social Work (required)
A minimum of one year as a practicing social worker in a health care or social service setting is required. Equivalent experience in a healthcare setting internship will be considered.

Responsible for addressing the psychosocial issues that arise in the community health setting through a range of focused social work interventions and practices. Services are provided upon referral by clinicians, nursing staff/physicians, community contacts, or by patients or their families. Services address those areas of psycho-social functioning that adversely affect or are affected by the patient's social determinants of health. Interventions include direct support of patients and families regarding emotional, financial, environmental, and safety issues, as well as appropriate referrals and coordination with other community agencies. The role is designed to assist patients, families, physicians, and other members of the healthcare team to work through an identified problem to positively affect the continuity of care and optimal outcome of healthcare services in the community setting.
Demonstrates knowledge about human behavior and theory; and about the psychosocial impact of disease on individuals and families as it relates to their age and stage of development.. Exhibits clear written and verbal communication skills.
Supports the mission and philosophy of the department as well as the Operating and Service Standards of Baystate Health.
Job Responsibilities:
1) Provides a comprehensive age-specific, psychosocial assessment of patients, including social, psychological, cultural, environmental, and financial information, and utilizes this information to develop a psychosocial treatment plan. Incorporates cultural and spiritual variables into the assessment of abuse/neglect for children, disabled adults, elders, and others. File reports with appropriate agencies as needed.
2) Ability to formulate treatment goals/plans with the patient and refer to other disciplines as needed. Supports patients' rights by treating their individual needs in the framework of their specific psychosocial, spiritual, developmental, and cultural variables.
3) Coordinates and documents the transition of the care plan toward the desired outcome, making recommendations and revisions to the plan as indicated. Make referrals, as needed, to social service/community agencies and programs. Communicates and involves the health care team, patient, and family throughout the performance of this process.
4) Maintains all appropriate records and follows the department documentation policy. Demonstrates a clear understanding of Electronic Health Record documentation systems. Documents on-call interventions as indicated.
5) Adheres to the department's Policies and Procedures. Meets productivity standards. Contributes to Ql activities when indicated to improve areas within clinical practice, or systems issues affecting the delivery of service. Shares in weekend and other special coverage needs as required by the department.
6) Collaborates with a multidisciplinary team to ensure appropriate post-clinic care plans are developed. Assists healthcare providers in understanding the impact of the psychosocial variables.
7) Demonstrates professional growth and development by conference participation, membership in professional societies, and other activities to increase knowledge about medical conditions, social work practice, and knowledge of patient resources. Attends in-service and education programs to maintain and enhance knowledge of current theory and practice and to maintain licensure. Seeks supervision and consultation from LICSW leadership as needed.
