Job Title: Registered Nurse (RN) – Case Management

Assignment Duration: 13 Weeks

Schedule: Rotating Day Shifts | 8:00 AM – 4:30 PM

• Every other weekend

• Rotating holidays
  
  Hours: 40 hours per week

Location: Hyannis, MA

Compensation

• Gross Weekly Pay: $2,723.62

Position Summary

We are seeking an experienced Registered Nurse – Case Manager to coordinate patient care across the continuum in an acute care setting. The RN Case Manager will be responsible for discharge planning, utilization review, and ensuring appropriate resource utilization while collaborating with interdisciplinary teams to support high-quality patient outcomes.

Key Responsibilities

• Coordinate and manage patient care plans from admission through discharge

• Perform utilization review using established criteria, including InterQual

• Lead discharge planning activities to ensure safe and timely transitions of care

• Collaborate with physicians, nursing staff, social workers, and ancillary services

• Communicate effectively with patients, families, and care teams regarding care plans

• Ensure compliance with regulatory, payer, and facility guidelines

• Maintain accurate and timely documentation in accordance with policies

Required Qualifications

• Active Massachusetts Registered Nurse (RN) license

• Minimum of three (3) years of acute care nursing experience

• Proven experience in Case Management, Discharge Planning, and Utilization Review

• Working knowledge of InterQual criteria

• Strong communication, organizational, and interdisciplinary coordination skills
