Responsibilities
- Assess residents and develop individualized physical therapy plans of care with functional short- and long-term goals.
- Perform evaluative and therapeutic procedures under physician orders, observing precautions and contraindications.
- Monitor resident progress, adjust treatment plans as needed, and document care in the Electronic Medical Record clearly and timely.
- Provide education to residents and caregivers; train on assistive devices and discharge requirements; coordinate with the care team.
- Collaborate with an interdisciplinary team, attend pertinent meetings, and communicate plan of care and discharge needs to the referring physician.
- Supervise assistant-level staff when appropriate; promote department standards, infection control, and a safe, clean clinical environment.

Requirements
- Active Physical Therapist license in Massachusetts (or applicable state)
- Bachelor’s degree in Physical Therapy; Master’s or Doctorate preferred
- Must have Skilled Nursing facility / long-term care experience
- Basic Life Support (BLS) certification preferred
- Strong verbal and written communication skills; ability to work collaboratively in an interdisciplinary team
- Ability to perform essential physical tasks in this setting, including lifting up to 50 pounds (two-person lift for heavier loads)
