Responsibilities
- Perform initial PT evaluations and develop individualized plans of care for residents in a SNF setting.
- Provide rehabilitation interventions to improve mobility, strength, balance, gait, and activities of daily living.
- Implement evidence-based treatment programs, monitor progress, and document outcomes in the charting system.
- Reassess patients regularly, adjust plans of care as needed, and coordinate with the interdisciplinary team.
- Participate in discharge planning and family/caregiver education as appropriate.

Requirements
- Active Physical Therapist license in Massachusetts or eligible for licensure in MA.
- Availability to commit to a 13-week travel assignment with 36 hours per week minimum.
- Experience in a skilled nursing facility or post-acute rehabilitation setting preferred.
- Strong documentation and communication skills; ability to collaborate with an interdisciplinary team.
- Ability to work in a SNF environment and adapt to a travel assignment schedule.
