We are hiring a Clinical Nurse Manager to provide personalized care management for patients with complex health needs. This role is 100% remote for Massachusetts residents, offering competitive pay, work-life balance, and the chance to make a meaningful difference in patient outcomes.

Key Responsibilities:

• Conduct clinical assessments and develop personalized care plans
• Coordinate care across providers, specialists, and support services
• Educate and engage patients to achieve treatment adherence and health goals

Requirements (Must-Have):

• Active RN license in Massachusetts
• 2+ years of clinical experience (inpatient, outpatient, or similar setting)
• Strong clinical assessment and communication skills

Nice-to-Have Experience:

• Case Management certification (CCM) or similar credential
• Experience in diabetes education or health coaching (e.g., Wellcoach)
• Familiarity with social determinants of health and reducing care disparities
