Sunbelt Staffing is hiring a full-time Home Health RN for a 13-week contract in Pittsfield, MA . This opportunity is ideal for an RN with home health experience who is ready to provide patient-centered care, perform thorough assessments, and implement effective care plans with a trusted home health agency.

Position Details:

• Location: Pittsfield, MA, and surrounding areas
• Company: Sunbelt Staffing, partnered with a Nationwide Home Health Agency
• Position Type: Full-Time Contract, Scheduled 40 hours per week - no guaranteed hours
• Contract Length: 13 Week Contract, with possibility of extension
• Schedule: Monday-Friday, 8 AM - 5 PM
• On Call: Typically one weekend per month, varies per branch
• Productivity: 30-32 per week, up to 35 patients
• Mileage: .48c visit to visit, 150-250 miles per week expected

Key Responsibilities:

• Conduct comprehensive patient assessments using the Oasis documentation system.
• Develop and implement individualized care plans tailored to each patient's specific needs.
• Utilize Homecare Homebase for accurate and timely documentation.
• Provide skilled nursing care and education to patients and their families in their homes.
• Coordinate with interdisciplinary team members to ensure continuity of care.
• Manage patient caseload and ensure timely visits.
• Participate in on-call rotation, including one weekend per month.

Qualifications:

• Active Massachusetts Registered Nurse (RN) License
• Active American Heart Association or American Red Cross BLS Certification
• At least 1 year of agency home health experience required
• Proficiency in Oasis and Homecare Homebase required
• Reliable transportation

Important Note: This contract role has limited training. Candidates must have at least one year of home health experience and be proficient in OASIS and Homecare Homebase (HCHB) to be considered.
