POSITION SUMMARY: The case manager (CM) will have a minimum of a Bachelor’s Degree with two years of supervised fulltime work experience with families with histories of substance use disorders/trauma, or a LADC II, or 5 years of experience providing counseling and/or case management services to families with histories of substance use disorders/trauma. The case manager will ensure that the Ummi’s Family Sober Living Program has appropriate linkages with the required community services, has experience in formulating individual and family service plans, and the ability to keep accurate records and submit required reports in a timely manner. The Case Manager provides support, advocacy, direction and guidance to families living in the program to achieve the goal of permanent housing. The Case Manager is responsible for providing comprehensive assessments, care planning, advocacy, and collaboration with multiple providers in order to assist families in navigating and accessing resources to achieve social and economic stability and secure affordable and sustainable housing. The Case Manager works as part of a multidisciplinary team and works closely with the other the Program Director, other Case Manager, Children’s Services Coordinator and Residential Recovery Specialists to meet the goals of each family and the program. Weekends, Holiday and shift rotation may be required. The work includes, but is not limited to, identifying and addressing barriers which may prevent families from obtaining permanent housing and developing an individualized plan for each family/individual in which those barriers are addressed. The Case Manager also is responsible for networking effectively with family members, physicians, courts, human service and community agencies and any other relevant persons or entities in order to promote the health and wellbeing of the family. The work may also entail accompanying families and/or individuals into the community to access various services.
Schedule: Monday to Friday from 4pm to 8pm
MINIMUM QUALIFICATIONS:
A. Education:
B.A. or B.S. degree preferred. 5 years of experience in case management/housing/substance disorder/trauma field may substitute for B.A/B.S. degree, in which case a high school diploma is required.
B. Experience:
B.A. or B.S. degree and one year of experience in related field, as specified above. High school diploma with at least 5 years of experience in related field, as specified above.
C. Knowledge/Skills:
- Excellent verbal and written communication skills.
- Excellent interpersonal skills.
- Knowledge of substance use disorder and trauma informed care.
- Knowledge of homeless and housing issues, including housing subsidies.
- Must be able to develop professional relationships with court personnel, counselors, medical personnel, landlords, property managers, and other community personnel to promote the health and wellbeing of families and assist to secure and maintain permanent housing.
- Must be able to work sensitively and effectively with people of diverse cultural backgrounds.
- Must have valid driver’s license.
- Must be willing to drive in Boston and surrounding communities.
- Must be willing to work a flexible schedule at times, including some evening hours.
- Knowledge of community resources.
- Ability to work both independently and as a team member.
- Ability to establish and maintain strong collegial relations with co-workers and service providers.
- Good computer literacy.

- Support and maintain a caseload of 12 families
- Conduct interventions as needed to attain positive outcomes.
- To accompany families to community venues related to wellbeing of family and housing search and placement.
- To perform duties that ensure the health, safety and well-being of individuals, families and staff, including working with other staff members to this end.
- To assist families in the implementation of Case Management Plans.
- To maintain appropriate records pertinent to individualized plan for each family and other records as necessary and as directed by the Program Director.
- To instruct and assist families to comply with DPH regulations and guidelines.
- To meet with all families on a weekly basis to review case management plans.
- Assist families in accessing appropriate services and resources for health care, career development, education and financial planning, housing etc.
- To attend staff meetings, management meetings, supervision meetings and other meetings or trainings as directed by the Program Director.
- To maintain current First Aid and CPR certificates.
- To establish and maintain professional, courteous communication at all times with all families, co-workers, colleagues, community representatives, medical personnel, legal personnel, DPH personnel, and all others involved in the families’ lives.
- To abide by all Dimock’s personnel policies and procedures.
- To represent Dimock’s Ruth Kelley Ummi’s Family Sober Living program to the public in a professional manner at all times.
- Other related tasks under the direction of the Program Director.

Salary Range: $18.88 to $26.66 per hour
