Oncology Survivorship Nurse Practitioner
Position Overview
The Oncology Survivorship Nurse Practitioner (NP) will provide comprehensive survivorship care, care coordination, and oncology-focused primary care services for patients across the cancer continuum. Working collaboratively with primary care providers, oncology specialists, behavioral health providers, community organizations, and academic medical centers, the NP will ensure patients receive coordinated, evidence-based care throughout diagnosis, treatment, survivorship, and palliative care. This position is Part-time between 16 to 20 hours per week.
The Oncology Survivorship NP will function as an Advanced Practice Registered Nurse (APRN), conducting independent patient assessments, developing treatment plans, ordering and interpreting diagnostic tests, prescribing medications and therapies, and managing survivorship-related conditions within the scope of Massachusetts APRN practice regulations and MFM Health policies.
The position will utilize national guidelines and best practices, including:
- American Society of Clinical Oncology (ASCO) guidelines
- National Comprehensive Cancer Network (NCCN) guidelines
- Oncology Nursing Society (ONS) survivorship recommendations
- Evidence-based survivorship care frameworks
- MFM Health clinical policies and procedures
Target Patient Populations
Newly Diagnosed Cancer Patients
- Establish an early relationship with patients following diagnosis.
- Serve as a liaison between primary care and oncology teams.
- Assist patients in navigating treatment plans, appointments, and support services.
- Promote continuity of care throughout the treatment journey.
- Provide education regarding diagnosis, treatment expectations, and supportive resources.
Patients Undergoing Active Treatment
Including patients receiving:
- Chemotherapy
- Immunotherapy
- Targeted therapy
- Hormonal therapy
- Radiation therapy
- Surgical oncology care
- Clinical trial participation
Responsibilities include clinical evaluation, symptom management, patient education, medication management, care coordination, and communication with treating oncology teams.
Patients Transitioning from Active Treatment
- Facilitate reintegration into primary care.
- Ensure appropriate surveillance, preventive care, and chronic disease management.
- Develop individualized survivorship care plans.
- Reduce the risk of patients becoming disconnected from primary care services.
Long-Term Cancer Survivors
Including:
- Childhood cancer survivors
- Adolescent and Young Adult (AYA) survivors
- Adult survivors
- Geriatric oncology patients
Patients Experiencing Treatment-Related Complications
- Evaluate, diagnose, manage, and coordinate referrals for acute and long-term treatment effects.
- Collaborate with specialty providers to optimize patient outcomes.
Patients Receiving Palliative Care
- Collaborate with palliative care teams to optimize quality of life.
- Assist with symptom management, advance care planning, and care coordination.
Core Survivorship Focus Areas
A primary responsibility of the Oncology Survivorship NP will be the identification, assessment, management, treatment, and referral of survivorship-related concerns, including:
- Sexual health and intimacy concerns
- Peripheral neuropathy and neurotoxicity
- Cognitive impairment ("chemo brain")
- Lifestyle modification and wellness planning
- Nutrition and exercise counseling
- Stress management and emotional well-being
- Anxiety and depression screening and treatment
- Lymphedema prevention and management
- Fatigue management
- Long-term and late effects of cancer treatment
- Fear of cancer recurrence
- Return-to-work and functional recovery challenges
- Bone health and osteoporosis prevention
- Cardiovascular risk management following cancer treatment
Additionally, all oncology patients will receive assessment and education regarding:
- Risk of cancer recurrence
- Risk of secondary malignancies
- Recommended surveillance and preventive care
- Health promotion and risk reduction strategies
Essential Duties and Responsibilities
Advanced Clinical Practice
- Perform comprehensive health assessments and survivorship evaluations.
- Diagnose and manage oncology-related and survivorship-related conditions.
- Order, perform, and interpret diagnostic testing within scope of practice.
- Prescribe medications, therapies, and supportive care interventions.
- Manage chronic conditions in collaboration with primary care providers.
- Monitor treatment-related complications and long-term health outcomes.
- Develop individualized care plans and survivorship treatment plans.
- Provide evidence-based preventive care and screening recommendations.
Patient Navigation and Care Coordination
- Coordinate care across oncology, primary care, specialty care, behavioral health, and community resources.
- Facilitate referrals and follow-up care.
- Assist patients through transitions between treatment phases.
- Ensure timely communication among care teams.
- Remove barriers to care and improve access to services.
Provider Support and Consultation
- Serve as a survivorship and oncology resource for MFM Health providers.
- Provide consultation regarding treatment-related complications and survivorship recommendations.
- Support evidence-based care planning and clinical decision-making.
- Assist providers in implementing survivorship best practices.
Education and Program Development
- Provide education to providers, staff, patients, and caregivers regarding survivorship care.
- Develop educational materials and survivorship resources.
- Support development of survivorship workflows and clinical pathways.
- Identify and lead quality improvement initiatives.
- Assist with program growth, outcome tracking, and reporting.
Community and Academic Collaboration
- Maintain collaborative relationships with community oncology practices and academic cancer centers.
- Promote seamless information sharing and coordinated patient care.
- Facilitate access to specialized oncology services and support programs.
- Represent MFM Health in survivorship and oncology-related initiatives.
Documentation and Compliance
- Maintain timely and accurate medical record documentation.
- Ensure compliance with regulatory requirements, payer guidelines, and quality standards.
- Participate in clinical quality, accreditation, and performance improvement activities.
Lifestyle & Structure
- Part-Time (20 hours per week)

Qualifications
Education
- Master's Degree or Doctorate from an accredited Nurse Practitioner program required.
- Graduate of an accredited Advanced Practice Registered Nurse (APRN) program.
Licensure and Certification
- Current unrestricted Massachusetts APRN license.
- Current Massachusetts Registered Nurse license.
- National Board Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP), Adult-Gerontology Nurse Practitioner (AGNP), or other applicable specialty certification.
- Current Massachusetts Controlled Substance Registration (MCSR), or ability to obtain.
- Current DEA registration, or ability to obtain.
- Current BLS certification required.
- ACLS certification preferred.
Experience
- Minimum of three (3) years of Nurse Practitioner experience preferred.
- Minimum of three (3) years of oncology, survivorship, hematology/oncology, primary care, women's health, or related clinical experience preferred.
- Experience managing complex chronic conditions and coordinating multidisciplinary care strongly preferred.
Knowledge, Skills, and Abilities
- Strong clinical assessment, diagnostic, and treatment planning skills.
- Knowledge of oncology survivorship principles and evidence-based guidelines.
- Ability to independently manage patient care within APRN scope of practice.
- Excellent communication and relationship-building skills.
- Strong organizational, documentation, and care coordination abilities.
- Ability to manage multiple priorities and complex patient populations.
- Commitment to patient-centered, team-based care.
- Proficiency with electronic medical records and population health management tools.
Salary This is a part-time position with a flexible schedule of 16–20 hours per week. Compensation will be determined and prorated based on the final agreed-upon weekly hours.
