MDS Coordinator

• Coordinate and complete MDS assessments in accordance with CMS, state, and facility guidelines
• Ensure accurate, timely submission of assessments and supporting documentation
• Collaborate with nursing leadership and interdisciplinary teams to support care planning and quality outcomes
• Monitor compliance with regulatory requirements related to MDS and reimbursement
• Participate in audits, quality improvement initiatives, and survey readiness activities
• Maintain up‑to‑date knowledge of MDS regulations and best practices

• Active nursing license required (RN strongly preferred)
• Minimum 1 year of MDS experience
• Prior experience in a skilled nursing / long‑term care setting
• Strong attention to detail and organizational skills
• Ability to work collaboratively with clinical and administrative teams
• Proficient in reading, writing, and following instructions in English
