HOME HEALTH PHYSICAL THERAPIST
Job Title: Physical Therapist – Home Health
Rate: Up to $2300.00 weekly
Location: Agawam, MA
Day (8H) - (5x8hr) - (8A - 5P)
Key Responsibilities
- Patient Assessment : Evaluate patients' physical function and limitations within their home environment to develop individualized treatment plans.
- Treatment Implementation : Provide therapeutic interventions aimed at improving mobility, strength, balance, and pain management.
- Education : Educate patients and caregivers on diagnoses, safety precautions, equipment use, and home exercise programs.
- Collaboration : Work with an interdisciplinary team, including nurses and occupational therapists, to create comprehensive care plans that address the patient's physical, emotional, and social needs.
- Documentation : Maintain accurate records of patient evaluations, treatment plans, and progress according to healthcare standards.
Education
- Required a degree in physical therapy (Bachelor’s or Master’s)
Experience
- Previous experience in home health care of similar setting is preferred.
Licenses/Certifications
- Must hold a valid state license to practice physical therapy
- Must be able to obtain and maintain CPR certification.
