Responsibilities
- Perform initial assessments and develop individualized treatment plans for residents
- Implement evidence-based physical therapy interventions to improve mobility, strength, and balance
- Reassess progress regularly, adjust plans of care, and document outcomes
- Collaborate with nursing and therapy teams; contribute to discharge planning and goal setting
- Maintain patient safety, apply infection control practices, and ensure HIPAA-compliant documentation

Requirements
- Active Physical Therapist license in Massachusetts or eligibility for licensure
- Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program
- Experience or preference for geriatric rehabilitation or skilled nursing settings
- Ability to work effectively in a multi-disciplinary, patient-centered environment
- Strong communication and documentation skills
