Job Type: Contract (13 Weeks)
Compensation
- Traveler Pay: $2,800 – $3,100 per week (based on experience)
- Local Pay: $2400 – $2640 per Week (based on experience)
Schedule
- Shift: Evening Shift (Rotating Days, Weekends & Holidays)
- Hours: 40 hours/week
- Shift Timing: 11:30 AM – 10:00 PM or 12:00 PM – 10:30 PM
- Schedule Type: 4 x 10.5-hour shifts
Job Summary
The Case Management RN is responsible for coordinating and facilitating patient care, ensuring efficient, high-quality, and cost-effective outcomes. This role focuses on discharge planning, utilization review, and collaboration with interdisciplinary teams to support continuity of care and optimize patient outcomes.
Responsibilities
- Perform daily screening of new admissions for discharge planning needs
- Conduct initial patient assessments within one business day using appropriate criteria tools
- Reassess patients regularly based on clinical condition and care needs
- Evaluate appropriateness of admissions and level of care using established guidelines
- Coordinate discharge planning and ensure continuity of care across settings
- Facilitate utilization review and communicate with third-party payers
- Manage discharge appeals and initiate required notifications
- Collaborate with physicians and interdisciplinary teams on care planning
- Identify high-risk patients and coordinate appropriate referrals and services
- Participate in multidisciplinary rounds and care conferences
- Ensure accurate documentation and compliance with policies and regulations
- Support reimbursement processes and assist with medical necessity documentation
- Advocate for patients and families regarding care plans and insurance matters
- Coordinate safe and timely patient transfers to other facilities or services
Required Qualifications
- Active Registered Nurse (RN) License – Massachusetts
- Minimum 3 years of acute care experience (within the last 5 years)
- Experience in discharge planning and utilization review
- Strong knowledge of InterQual or similar criteria systems
- Experience with Epic EMR
- Excellent communication, organizational, and multitasking skills
- Ability to work independently in a fast-paced environment
Education & Certifications
- BLS Certification (required)
- BSN preferred
- Case Management Certification (CCM/CPUM) preferred
