Overview: A major Florida-based acute care hospital system is seeking an experienced Manager of Case Management to lead utilization review, discharge planning, and care coordination efforts. Key Responsibilities:

• Lead and manage case management operations and discharge planning workflow
• Collaborate with physicians, nursing, and payer partners to ensure timely transitions of care
• Ensure compliance with regulatory guidelines, payer requirements, and hospital policies
• Drive performance improvement initiatives to optimize LOS, care coordination, and throughput
• Oversee staffing, resource allocation, and departmental performance metrics

Qualifications:

• Active RN license in Florida or Compact state
• Minimum 1 year of case management leadership experience in an acute care hospital setting
• Bachelor’s degree required; Master’s degree preferred
