Job Title: Clinical Social Worker (LCSW / LICSW)

Location: Hyannis, MA

Job Type: Contract – 26 Weeks

Shift: 8:00 AM – 4:00 PM

Schedule: Rotating Days with Rotating Weekends

Hours: 40 Hours/Week
Occasional holiday coverage required

About the Role:
Seeking an experienced Clinical Social Worker to provide psychosocial support, crisis intervention, and discharge planning services within an acute care hospital setting.

Key Responsibilities:

• Conduct psychosocial assessments for patients and families

• Provide counseling, advocacy, and crisis intervention services

• Support patients with mental health, substance abuse, and social needs

• Develop individualized discharge plans

• Coordinate referrals for rehabilitation, long-term care, and community services

• Assist with financial assistance and community resource coordination

• Collaborate with interdisciplinary healthcare teams

• Maintain accurate EHR documentation

• Participate in quality improvement and departmental initiatives

Requirements & Preferred Skills:

• Master’s Degree in Social Work (MSW) required

• Active Massachusetts LCSW or LICSW license required

• CPI/MAB certification required within 1 month of hire

• Strong EHR and computer skills required

• Acute care hospital experience preferred

• Experience with crisis intervention and discharge planning preferred

• Behavioral health and substance abuse counseling experience preferred

• Strong communication, organization, and multitasking skills

• Ability to work independently in fast-paced environments
