Case Manager, Acute Treatment Services (ATS) – 30 Hour Part Time Weekend Role
• Hours: 7:00 AM – 3:30 PM from Friday to Monday. You must be able to work this schedule every week.
• Pay: $25/hr – $32/hr dependent on related experience, education, and licensure.
Are you looking for meaningful part-time work in case management during the weekends? This role is ideal for candidates currently completing a graduate degree in a Social Work related field and looking to gain hands-on clinical experience.
Summary/Objective
The Case Manager is part of the clinical team providing care coordination and discharge planning for individuals receiving residential substance use treatment in Acute Treatment Services (ATS). This role supports clients in identifying needs, accessing services, and preparing for successful transition into the next level of care. The Case Manager works closely with clinicians, recovery specialists, and community providers to ensure coordinated care and successful treatment outcomes.
Essential Functions
• Engage clients in treatment planning and support progress toward individualized goals.
• Conduct assessments to identify client needs, strengths, and barriers to recovery.
• Meet with clients regularly to provide care coordination and discharge planning.
• Develop care plans in collaboration with clients and the multidisciplinary treatment team.
• Coordinate referrals and linkages to community resources including housing, employment, healthcare, and financial supports.
• Collaborate with external providers including courts, primary care providers, and behavioral health specialists.
• Facilitate psychoeducational groups as assigned.
• Coordinate discharge and aftercare planning to ensure continuity of care.
• Conduct post-discharge follow-up as appropriate.
• Assist the clinical team in crisis management when necessary.
• Maintain professional and respectful boundaries with clients.
• Complete documentation accurately and in accordance with Joint Commission, Dimock, and regulatory standards.
• Attend required staff meetings and trainings.
• Adhere to all organizational, federal, and state regulations.
• Perform other duties as assigned.

• Intermediate computer proficiency including Microsoft Word, Excel, and electronic documentation systems.
Work Environment / Work Standards
• This position operates primarily in a clinical residential treatment setting.
• Adheres to departmental and organizational policies and regulatory requirements.
Physical Demands
• Light physical activity including lifting or carrying up to 20 pounds.
Position Type/Expected Hours of Work
• Part-time weekend role working Friday through Monday from 7:00 AM – 3:30 PM. Additional weekday hours may be available based on program needs.
Required Education
• Bachelor’s Degree in Human Services, Psychology, Sociology, Health Services, or a related field.
• At least one year of experience working in a substance use treatment program.
• A Master’s Degree in Social Work (MSW) or related field is highly preferred. Candidates currently enrolled in a graduate program are strongly encouraged to apply.
Preferred Education/Experience
• Two or more years of experience working with individuals experiencing substance use disorders, mental illness, or homelessness.
