Job Opportunity: Medical Director - Geriatric Medicine

Mercy LIFE Massachusetts a member of Trinity Health is seeking a dedicated full-time Medical Director to lead our innovative Program of All-Inclusive Care for the Elderly (PACE) in West Springfield MA. This leadership role offers an exciting opportunity to provide comprehensive medical oversight and quality care coordination for seniors with chronic care needs helping them maintain independence while living safely in their communities.

Job Responsibilities:

• Conduct history and physical assessments on new participants and perform event-based and periodic reassessments.
• Develop medical treatment plans and provide necessary medical services across various participant residence locations.
• Interview and evaluate applicants to PACE Organization and their caregivers for Intake and Assessment decisions.
• Discuss participants medical conditions and prognosis with them and their caregivers to aid in the planning of future care.
• Collaborate in developing nursing treatment programs for participants.
• Participate actively as a member of the PACE Organization interdisciplinary team advising on diagnoses and treatment options.
• Educate participants and families regarding health maintenance and chronic medical conditions in cooperation with nursing staff.
• Supervise the healthcare and treatment programs established by the interdisciplinary team.
• Perform physical examinations order and evaluate laboratory and diagnostic tests and record results methodically.
• Participate in provider on-call rotation as required.

Qualifications:

• Current and active plenary physician license to practice in Massachusetts.
• Board Certification in Internal Medicine or Family Practice is required; Board Certification in Geriatrics is preferred.
• Previous supervisory experience is required.
• A minimum of one (1) year of documented experience working with a frail or elderly population is required.
• Effective skills in physical assessment and chronic disease management for frail older adults are required.
• A desire to work in a creative alternate healthcare setting is essential.
• Able to manage changing priorities according to participant needs.

License and Certification Requirements:

• Current and active plenary physician license to practice in Massachusetts REQUIRED.
• Board Certification in Internal Medicine or Family Practice REQUIRED; Board Certification in Geriatrics PREFERRED.
• CDS Certification and DEA Registration REQUIRED.
• Previous supervisory experience REQUIRED.
