Registered Nurse (RN)
Functions:
- Assessment:
- Collects, validates, synthesizes and records objective and subjective data
- Prioritizes data collection based on resident needs
- Identifies actual or potential problems or needs based on resident needs
- Care Planning:
- Establishes realistic and measurable short and long-term goals consistent with the delivery of resident care
- Develops individualized interventions to achieve goals
- Conducts a systematic and ongoing evaluation of resident outcomes
- Reviews and revises plan of care as indicated
- Managing Resident Care:
- Performs nursing functions and provides care within scope of practice
- Coordinates and delegates as appropriate
- Administers medications and performs treatments per physician orders
- Communicates and documents interventions and resident response as needed
- Supervises co-workers to ensure that delegated tasks and interventions are implemented appropriately and per policy
- Evaluates and adjusts the plan of care to achieve resident goals and minimize hospitalizations
- Communicates recommendations for discharge to the attending physician, resident and family
- Prepares resident family for discharge including resident and family education
- Participates in shift to shift communication between incoming and outgoing nursing staff
- Performs drug count when beginning and ending shift and whenever necessary
- Collaborates with physicians in rounds and examination of residents as needed
- Collaborates and coordinates with other departments to provide timely effective care consistent with resident needs, choices and preferences
- Contributes to an environment that is respectful, team oriented and responsive to the concerns of co-workers, residents and families
- Enhances nursing practice by attending all mandated in-service programs
- Assists with the orientation of newly hired nursing employees
- Promotes a culture of safety
- Maintains confidentiality and protects sensitive Protected Health Information (HIPAA) at all times
- Participates in quality improvement activities (QAPI) as requested
- Stays and works beyond scheduled shift if needed to meet state staffing requirements and or needs of residents
- Must have the ability to work a schedule that requires an every other weekend commitment
Qualifications:
Graduate of an accredited RN school of nursing. Valid RN license in the state employed. Valid CPR certification. Must be able to read, write, speak and understand the spoken English language to ensure the safety and wellbeing of our residents and visitors at the work site when responding to their medical and physical needs. Prior skilled nursing experience preferred.
