LPN or RN – Full-Time, No Weekends – Concierge Primary Care

Discover Health is seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our growing concierge primary care team in Waltham, MA .

You’ll be supporting three board-certified internal medicine physicians who value trust, teamwork, and relationship-driven care. This is a Monday–Friday, full-time, no weekends opportunity within a supportive clinical team that values quality over quantity.

What You’ll Do:

• Be a first point of contact: Answer incoming patient calls and portal messages with compassion and clinical insight

• Perform comprehensive clinical assessments and determine appropriate levels of care

• Use sound judgment and nursing expertise to triage and support patient needs

• Collaborate closely with physicians and staff to ensure continuity and excellence in care

• Access and document patient information in Epic (EMR) with accuracy and clarity

• Support our mission of high-touch, patient-centered healthcare

Schedule & Location:

• Full-Time (36.5 hours/week)

  • Monday – Thursday: 8am–4pm

  • Friday: 8am–3pm

• Onsite only (Waltham, MA)

• Free parking available

• No weekends or federal holidays

✅ Requirements:

• Licensed LPN or RN in the state of Massachusetts

• Experience using Epic EMR (Required)

• 2+ years of clinical experience preferred

• Strong communication and problem-solving skills

• A passion for personalized, relationship-based care
