Community Health Worker- Ryan White Program
Salary for this position: $18.00 - $21.00/Hour

We are currently hiring a Community Health Worker – Ryan White Program based in Milford. As an integral member of the care management team this role will work as a member of a Ryan White team and will provide a range of client-centered activities focused on improving health outcomes in support of the HIV care continuum. This position requires flexibility and may vary from day- to-day to meet members where they are. Outreach methods may vary based on the needs of the organization and may include telephonic or in person in a variety of potential settings such as but not limited to, the community, home facility or health center. Essential Functions:Coordinates care for HIV patients per Ryan White Program guidelines to ensure quality care.
Assesses Social Determinants of Health (SDOH) for referred patients and develops intervention plans based on identified needs.
Provides basic HIV education, screening, brief interventions, and referrals per health center protocols.
Assists patients in navigating the health care system and applies for HIV Drug Assistance Program (HDAP) as needed.
Participates in outreach and population health efforts, such as missed appointment follow-ups and preventive care reminders.
Supports patients with psychosocial barriers to care and connects them to services promoting treatment adherence and self-sufficiency.
Completes documentation and reporting requirements, including data entry into Ryan White and E2 Boston systems.
Engages in community outreach activities and may perform specialized tasks like home or hospital visits.
Fulfills grant-related responsibilities and funding requirements for the role.
- Minimum 1 year of work or volunteer experience in human services or healthcare with demonstrated knowledge of or ability to work with the targeted community.
- Basic knowledge of Microsoft Office and ability to learn the health center’s Electronic Medical Record and other applications.
- Excellent written and oral communication skills.
- Successful completion of the CHW core competency training within 1 year of employment.
