Mobile Respite Counselor
If you are passionate about supporting individuals on their path to recovery and bring strong skills in counseling, case management, and collaboration, we invite you to join our Westfield Respite team as a Mobile Counselor. In this role, you will provide direct counseling and case management support to individuals served, guiding them through enrollment and discharge processes, developing individualized action plans (IAPs), and coordinating aftercare planning. You will implement a range of behavioral interventions, facilitate meaningful activities, and connect individuals to resources that support their recovery goals.
What You’ll Do:
As a Mobile Counselor, you will provide direct counseling and case management support to individuals in a respite setting, helping them stabilize, build skills, and work toward their recovery goals. You’ll play a key role in coordinating care, implementing individualized plans, and connecting individuals to essential services and community resources.
Key Responsibilities:
- Conduct clinical assessments and implement appropriate treatment or referral responses using the least restrictive approach
- Monitor and assess mental status, social functioning, and risk levels, responding proactively to individual needs
- Collaborate with family members, providers, and support systems to develop a comprehensive understanding of each individual’s situation
- Develop and implement Individualized Action Plans (IAPs) and support aftercare planning
- Provide counseling, behavioral interventions, and skill-building activities aligned with recovery goals
- Facilitate referrals to community resources and support services
- Assist with daily living needs, including budgeting, organization, health, and wellness support
- Support individuals with housing applications, financial entitlements, and securing stable living arrangements
- Provide transportation to appointments and recovery-related services as needed
- Complete all required documentation in a timely and accurate manner, including assessments, progress notes, incident reports, and discharge summaries
- Participate in team meetings, trainings, and ongoing professional development opportunities.

Hourly Rate: $21.50 - $25.27
