Job Summary

"Navitas Healthcare, LLC" is seeking a compassionate and skilled Registered Nurse (RN) – Home Health to deliver high-quality nursing care to patients in a home-based setting. The ideal candidate will conduct detailed assessments, develop individualized care plans, and collaborate with interdisciplinary teams to ensure safe, effective, and patient-centered care. This role requires strong clinical judgment, excellent communication skills, and the ability to work independently while managing multiple patient needs.

Key Responsibilities

• Conduct comprehensive patient assessments, including physical, psychosocial, and environmental evaluations
• Gather and analyze patient health history to ensure safe and effective care delivery
• Develop, implement, and update individualized care plans based on clinical findings
• Determine visit frequency and resource utilization based on patient needs
• Monitor and evaluate patient progress, ensuring alignment with care goals and quality standards
• Document all patient care activities accurately and in a timely manner
• Participate in interdisciplinary case conferences and evaluate plan effectiveness
• Provide supervision and guidance to Home Health Aides as required
• Communicate changes in patient condition to physicians and care teams promptly
• Complete all admission, visit, and discharge documentation within required timelines
• Obtain and clarify physician orders related to patient care
• Coordinate patient discharges and ensure proper follow-up care
• Connect patients with appropriate community resources when needed
• Maintain compliance with care standards, policies, and regulatory guidelines
• Perform additional duties as assigned

Qualifications

• Active Registered Nurse (RN) license in Massachusetts
• Minimum 1 year of nursing experience (or 6 months with BSN)
• Current CPR certification
• Ability to pass pre-employment medication competency assessment
• Home health or community-based care experience preferred
• IV therapy experience preferred
• BSN preferred
• Strong documentation and computer proficiency
