Healthcare Case Manager – RN

We’re seeking a Healthcare Case Manager to deliver coordinated, patient-centered care for members with complex health needs. This role is fully remote for Massachusetts-based RNs who want to combine clinical expertise with care management, making a real impact while enjoying schedule flexibility.

Key Responsibilities:

• Perform patient assessments and develop individualized care plans
• Coordinate care across physicians, specialists, and support services
• Guide, coach, and educate patients on health management and treatment adherence

Requirements (Must-Have):

• Active RN license in Massachusetts
• Minimum 2 years of nursing experience (inpatient, outpatient, or similar setting)
• Strong communication and clinical assessment skills

Nice-to-Have Experience:

• Case Management certification (CCM)
• Background in diabetes education or health coaching (e.g., Wellcoach)
• Experience reducing care disparities and addressing social determinants of health

Required Skills:

• Patient assessment & individualized care planning
• Patient education & engagement
• Verbal and written communication
