Prime Home Health and Companion Care is seeking compassionate Certified Nursing Assistants (CNAs) and Home Health Aides (HHAs) to support elderly, disabled, and chronically ill clients in their own homes. As a member of our team, you’ll provide essential care that includes personal care assistance, emotional support, and helping to create a safe, clean environment for each client.
Client Location: Southbridge, MA (potential for additional shifts with clients in nearby areas)
Shift Details: Sun. - Fri. 4:30 - 8:30 p.m. and Sat., Sun., Wed. 8:00 a.m. - 1:00 p.m.

Key Responsibilities
- Personal Care : Assist with daily activities, including bathing, dressing, grooming, and toileting.
- Emotional Support : Provide companionship and emotional encouragement, helping clients maintain a positive outlook.
- Mobility Assistance : Aid clients with mobility using Hoyer lifts and Sit-to-Stand devices (experience preferred but not required).
- Follow Care Plans : Work closely with nurses, therapists, and doctors to provide care as outlined in individual care plans.
- Maintain Environment : Keep the home environment clean, safe, and supportive of each client’s needs.
- Documentation : Accurately record patient care and report any changes in condition to the care team.
Requirements
- Certification : Current HHA certification.
- License : Valid driver’s license, reliable transportation, and proof of auto insurance.
- Experience : Prior experience with Hoyer lifts and Sit-to-Stand devices is beneficial but not required.
- Skills : Compassionate, reliable, patient, and committed to helping others.
