Position Overview

We are seeking a caring and motivated Registered Nurse (RN) to join our growing Apex AFC team. The RN will provide oversight, clinical assessment, and support to members and caregivers enrolled in our Adult Foster Care Program. This is an excellent opportunity for nurses passionate about community-based care and person-centered support.

Responsibilities

· Conduct initial and ongoing in-home nursing assessments for AFC members.

· Develop, implement, and review individualized care plans according to MassHealth regulations.

· Provide caregiver training, supervision, and ongoing health education.

· Monitor members’ health conditions and communicate updates with healthcare providers.

· Maintain accurate, timely, and compliant nursing documentation.

· Participate in interdisciplinary meetings and quality improvement initiatives.

· Ensure compliance with Apex AFC and MassHealth standards of care.

Qualifications

· Current and valid Massachusetts RN license (required).

· Minimum of 1 year of clinical experience (AFC, home care, or community nursing preferred).

· Strong interpersonal and communication skills.

· Ability to work independently, manage home visits, and meet documentation deadlines.

·Valid driver’s license and reliable transportation.

Workplace

Our administrative office is located in Malden, MA. The role involves both office-based and community (home visit) work within the surrounding areas.
