Job Summary
"Navitas Healthcare, LLC" is seeking a compassionate and skilled Physical Therapist with experience in a Skilled Nursing Facility (SNF) setting to join a dedicated rehabilitation team. This role focuses on helping residents achieve maximum functional independence and enhance quality of life through individualized therapy plans and collaborative, patient-centered care.
Key Responsibilities
- Evaluate residents and develop individualized treatment plans with measurable short- and long-term goals.
- Provide therapeutic interventions to improve mobility, strength, balance, and functional independence.
- Collaborate with physicians and interdisciplinary teams to ensure coordinated and effective resident care.
- Monitor patient progress and adjust treatment plans as needed to achieve optimal outcomes.
- Accurately document evaluations, treatments, and progress in the electronic medical record (EMR) system.
- Educate residents and caregivers on treatment plans, prescribed exercises, and use of assistive devices.
- Ensure compliance with Medicare, Medicaid, and other regulatory documentation requirements.
- Supervise and collaborate with Physical Therapist Assistants (PTAs) as appropriate.
- Participate in care planning meetings, discharge planning, and quality improvement initiatives.
- Maintain a safe, clean, and organized therapy environment while adhering to infection control protocols.
Required Qualifications
- Bachelor’s degree in Physical Therapy (Master’s or Doctorate preferred).
- Active Physical Therapist (PT) license in the state of Massachusetts.
- Prior experience in a Skilled Nursing Facility (SNF) setting.
- Strong clinical assessment and patient care skills.
- Knowledge of rehabilitation techniques, modalities, and assistive devices.
- Ability to work collaboratively within an interdisciplinary team.
- Excellent communication, documentation, and time management skills.
- Ability to manage a caseload in a fast-paced healthcare environment.
- Proficiency in electronic medical records (EMR) systems.
Preferred Qualifications
- Experience working in high-volume or post-acute care environments.
- Strong commitment to patient-centered care and continuous professional development.
