Qualifications
- Conduct health assessments and screenings in community settings
- Provide health education and preventive care to individuals and families
- Administer vaccines, medications, and treatments as needed
- Collaborate with public health agencies, social workers, and community leaders
- Develop care plans and provide case management support
- Monitor and address community health concerns, including outbreaks and chronic conditions

- Minimum 2 years of recent Community Health or Public Health experience
