- Hire, train and coach staff, ensure all program staff training requirements are met and maintained. Provide ongoing supervision, feedback, and performance support to team members.
- Develop and implement Individual Support Plans for each person served in collaboration with the clinical team. Train and monitor staff to ensure consistent, high-quality care.
- Ensure quality health care for each individual - Promote a healthy lifestyle within the home. Ensure timely scheduling and completion of all routine and follow-up medical appointments. Monitor that all medical and clinical recommendations are followed by all staff. Collaborate with all service providers; Nurses, Clinicians and Day Programming to meet behavioral and medical needs.
- Build and maintain positive relationships with family members and guardians. Communicate proactively and professionally regarding individual care. Coordinate and prepare for family and team meetings as needed. Train staff in best practices for working with families.
- Ensure records are updated as required (medication lists, doctor’s orders and medical documentation). Oversee medication orders and administration through daily and weekly checks.
- Provide emergency and on-call coverage as needed. Respond to incidents, manage call-outs, schedule per diem staff, complete on-call reports, and provide shift coverage if necessary.
- Bachelor's degree in Human Services or a related field may substitute for required experience.
- Supervisory and administrative experience in a residential or human services setting with a proven ability to foster a supportive and collaborative environment for staff and Individuals.
- Candidates must be able to read, write, and comprehend English to accurately follow the Individual Service Plan (ISP), document services provided, and communicate effectively with individuals, families, and healthcare professionals.
- Strong organizational and multitasking abilities; skilled in problem-solving
- MAP (Medication Administration Program) Certification; training is offered by Waystone to acquire certification within 6 months of employment
- CPR/First Aid; training is offered by Waystone to acquire certification within 3 months of employment
- Valid driver’s license in good standing with at least one year of driving experience. Must be able to drive and train staff on the use of wheelchair-accessible vans.
- Proficiency with Microsoft Office and general computer literacy, including experience with data management systems and recordkeeping platforms, such as HCSIS, Ehana, and Relias.
