Roles & Responsibilities

• Evaluate residents' physical therapy needs and develop personalized treatment plans
• Implement therapeutic exercises and interventions to improve mobility, strength, and function
• Document patient progress and adjust treatment plans to achieve optimal outcomes
• Educate residents and their families on home exercise programs and fall prevention
• Collaborate with nurses, occupational therapists, and other team members to provide holistic care
• Participate in care planning meetings and contribute to discharge planning

Qualifications

• Active Physical Therapist license in Massachusetts (or eligibility to obtain)
• Graduation from an accredited physical therapy program
• New graduates welcome to apply; no prior experience required
• CPR certification (BLS) preferred
• Strong communication and interpersonal skills
• Compassion for working with the elderly population in a skilled nursing setting

Knowledge & Skills

• Knowledge of physical therapy principles, modalities, and therapeutic exercise
• Skilled in patient assessment and treatment planning
• Proficient in documentation and electronic medical records (EMR)
• Excellent verbal and written communication
• Empathy and patience when working with residents
• Team collaboration and interdisciplinary care coordination
