RN – Home Visits (Adult Foster Care Program) – $450/day

Schedule
- 2 days per month (weekday visits only – no weekends)
- Start time: 7:00 AM (departing from Lynn, MA)
- End time: approximately 3:30–4:00 PM
- Average workday: 8–9 hours including travel, charting, and documentation
- Average number of visits per day: 6 (may vary slightly depending on member scheduling)
Compensation
- Base compensation: $50 per visit × average 6 visits/day = $300/day
- Mileage reimbursement: $0.70/mile (IRS rate)
- Average daily mileage: ~110 miles
- Includes $20 gas allowance per day
- Total daily pay with efficiency/performance bonus: ≈ $450 per day
- Equivalent to $50/hour × 9-hour workday -
- Per diem rate includes visit pay, mileage reimbursement, gas allowance, and a daily performance bonus.
- Position Type: 1099 Independent Contractor (not W-2 employee)
Route Overview
- Routes start and end in Lynn, MA .
- Service areas include North Shore, MetroWest, and Worcester County (visits vary monthly).
- Routes are optimized for efficiency and minimal drive time.
Responsibilities
- Conduct in-home nursing visits for Adult Foster Care (AFC) members.
- Complete clinical assessments, medication reconciliation, and care plan updates.
- Collaborate with caregivers, care managers, and the RN Clinical Manager.
- Document findings accurately in compliance with MassHealth AFC requirements.
- Communicate significant member changes promptly to the RN supervisor.
Requirements
- Active Massachusetts RN License in good standing.
- Valid driver’s license and reliable personal vehicle required.
- Minimum 1 year of nursing experience (AFC, home care, or geriatrics preferred).
- Excellent communication, assessment, and documentation skills.
- Must be legally authorized to work in the United States.
- Must pass background checks: CORI, SORI, OIG exclusion, and professional reference verification.
- Must provide health records (including immunization documentation and TB/PPD screening).
- Ability to work independently, maintain confidentiality, and complete required reports promptly.
- Professional references required.
