Job Title: Registered Nurse – Case Management
Location: Hyannis, MA 02601
Type: 13-Week Contract | Full-Time
Shift: Day Shift | 8:00 AM – 4:30 PM | 42.5 hours per week
Schedule: Rotating Days | Every Other Weekend | Rotating Holidays
Weekly Pay: $2,968.14 (gross estimate)
Position Overview:
We are seeking an experienced and dedicated Registered Nurse (RN) – Case Management professional to join a leading healthcare organization in Hyannis, MA. The ideal candidate will be responsible for coordinating patient care, discharge planning, and utilization review to ensure quality outcomes and cost-effective care delivery.
Key Responsibilities:
Conduct comprehensive assessments and develop individualized discharge plans.
Collaborate with interdisciplinary teams to coordinate patient care across the continuum.
Perform utilization review using InterQual® or similar clinical criteria.
Communicate effectively with physicians, patients, families, and payers to ensure continuity of care.
Facilitate timely and safe patient transitions from the hospital to appropriate post-acute settings.
Monitor and document patient progress, ensuring compliance with all regulatory and facility standards.
Provide education and support to patients and families regarding care options and available resources.
Qualifications:
Education: Graduate of an accredited School of Nursing; BSN preferred.
License: Active and unrestricted Registered Nurse (RN) license in the state of Massachusetts.
Experience: Minimum of 3 years of recent acute care experience (within the past 5 years) with strong background in discharge planning and utilization review.
Knowledge & Skills:
Proficient in InterQual® or equivalent review systems.
Strong organizational, communication, and time management skills.
Ability to work independently and manage multiple priorities.
Excellent interpersonal and negotiation abilities.
Competence in computer systems and clinical documentation.
Certifications:
BLS (Basic Life Support) – required.
Case Management Certification (CCM, ACM, or CPUM) – preferred.
