Position Overview
We are seeking a Registered Dietitian II to join our dynamic PACE interdisciplinary team. In this vital role, you will provide comprehensive nutritional assessments and develop individualized nutrition plans that support the overall health and wellness of our participants. The Registered Dietitian plays a key role in chronic disease management and works collaboratively with the care team to promote participant independence and safety in the community.
Key Responsibilities
• Conduct nutritional assessments and develop tailored nutrition plans for PACE participants
• Provide nutrition counseling to support healthy eating habits and chronic disease management
• Collaborate with the interdisciplinary team to integrate nutritional care into each participant's overall plan of care
• Monitor and adjust nutrition plans based on clinical outcomes and participant needs
• Participate in team meetings and contribute to holistic care planning
• Provide home-based visits as needed to support participant goals and safety
Qualifications
• Master's of Science from an accredited institution
• Valid Massachusetts R.D. license
• 1 year experience in geriatrics required; 1 year experience in clinical or community nutrition preferred
• 1-2 years in ambulatory setting strongly preferred
• Ability to transport between sites
• Computer literacy; Epic experience preferred
