Overview
Physical Therapist, General – Pittsfield, MA, United States. This travel, temporary assignment in a home health hospice setting begins 08/24/2026 for 13 weeks. Guaranteed 40 hours per week with daytime shifts (40 hours). Weekly pay range: $2,299 to $2,395. Setting is home health care focused on hospice services.

Responsibilities
- Provide direct physical therapy services to home health patients, including assessment, treatment planning, and ongoing rehabilitation.
- Develop, implement, and adjust the patient’s plan of care in collaboration with the interdisciplinary team.
- Utilize therapeutic modalities and rehabilitation equipment; apply splints, orthoses, prostheses, wheelchairs, and ADL equipment as appropriate.
- Document assessments, interventions, and outcomes; ensure compliance with home health documentation standards and regulatory requirements.
- Educate patients and families on safety, exercise programs, activities of daily living, and caregiver techniques.
- Coordinate care with other providers; participate in care conferences and contribute to the overall patient care plan.
- Travel to patient homes and maintain appropriate equipment; manage time effectively in a home-based setting.

Requirements
- Massachusetts Physical Therapist license (active)
- Home health experience with proficiency in OASIS (certified home health experience)
- 2+ years of combined acute hospital care and home health experience (preferred)
- Valid driver’s license and access to reliable personal vehicle
- Active American Heart Association BLS certification
- Ability to work independently with minimal supervision in a home health setting
