Join a growing home health team as a Patient Care Manager , where you'll play a critical role in ensuring high-quality patient care and supporting clinical excellence across the organization. This leadership position combines patient care oversight, staff supervision, and quality improvement initiatives to help deliver outstanding outcomes for patients in the community.

• Competitive salary of $90,000–$100,000
• Full-time schedule: Monday–Friday, 8:00 a.m.–5:00 p.m.

• Provide clinical supervision to ensure patient care meets professional standards, agency policies, and regulatory requirements
• Coordinate and oversee the delivery of patient care services
• Supervise and support clinical staff to ensure consistent, high-quality care
• Maintain effective communication with patients, caregivers, referral sources, and both field and office teams
• Drive quality outcomes by achieving departmental goals and contributing to performance improvement initiatives
• Serve as a role model for professionalism and organizational values
• Ensure compliance through required training, accurate reporting, and participation in audits and reviews
• Promote a safe work environment by following established safety protocols
• Support staff development through coaching, mentorship, and continuing education opportunities

• Current Registered Nurse (RN) license in the practicing state
• Experience in a home health or home care setting
• Previous management or supervisory experience preferred, with at least two years in a leadership capacity
• Knowledge of professional nursing standards and best practices
• Understanding of Medicare Conditions of Participation and applicable federal, state, and local regulations
• Strong leadership, communication, and organizational skills
• Commitment to quality patient care, compliance, and team development
