Position Summary:
As a Social Worker, you will play a vital role in addressing the emotional, social, and psychological needs of our residents. You will work closely with an interdisciplinary team to ensure residents' well-being and support them through transitions in care, life events, and family dynamics.

Key Responsibilities:

• Participate in and help coordinate interdisciplinary care plan meetings.
• Conduct and document psychosocial assessments and progress notes.
• Provide support and counseling to residents and families during admission, adjustment, and throughout their stay.
• Assist with discharge planning, PASRR evaluations, and service coordination.
• Serve as a resident advocate and connect families with appropriate community resources.
• Ensure timely and accurate documentation in compliance with regulatory standards.
• Maintain confidentiality and uphold resident rights at all times.

Qualifications:

• Licensed in Massachusetts as an LSW, LCSW, LICSW, or LSWA.
• Bachelor’s degree in Social Work from an accredited program required; Master’s degree preferred.
• Nursing home or long-term care experience strongly preferred.
• Familiarity with MDS 3.0 assessments, care planning processes, and PASRR evaluations is highly desirable.
• Excellent interpersonal skills and the ability to work effectively in a team environment.
• Empathy, patience, and strong problem-solving abilities.
