Andover Forest Post Acute Care Center is seeking an experienced RN MDS Specialist to join our team in a full-time, salaried, on-site leadership role within our skilled nursing facility.
This is a high-impact position focused on driving clinical accuracy, reimbursement optimization, and interdisciplinary care planning. You will serve as a key contributor to both clinical outcomes and facility performance , working closely with nursing leadership and the IDT.
What You'll Own- Full oversight of the MDS process , including scheduling, coordination, and completion
- Ensuring accuracy, timeliness, and compliance with all CMS and state requirements
- Driving PDPM performance and reimbursement optimization
- Reviewing clinical documentation to support maximum appropriate case mix index (CMI)
- Leading and participating in interdisciplinary care plan development
- Acting as a resource to nursing and clinical teams on documentation and regulatory standards
- On-site leadership presence with direct influence on outcomes

- Active RN license (Massachusetts)
- Demonstrated experience with MDS, PDPM, and SNF reimbursement

- $102,000 - $108,000/year (DOE)
- Compensation growth tied to facility census and performance
- Quality Measure Bonus: up to $7,500/year
