The Pharmacotherapy Services program has been in place since 2006, providing direct patient care in the ambulatory setting at all adult Primary care sites and some medical specialty sites at CHA. We currently have 20 clinical pharmacists, 2 specialized pharmacy technicians and 2 patient liaisons to support the patient care services we offer. Care delivery is a hybrid of telehealth and face to face in the clinic setting and via home visits to some of our patient population. We support Population Health initiatives related to medication management and consistently improve quality metrics and patient satisfaction. Our current services include anticoagulation, diabetes, hypertension, respiratory, smoking cessation, complex medication recommendations, fall prevention, infectious disease, pain management, psychiatry and immunizations/travel health. Clinical Pharmacists precept our APPE students, PGY-1 residents and collaborate with medical colleagues across the health system. Additionally, our team supports new service development, P & T projects, in-services and continuing education.
Job Purpose: Under the general supervision of the Director, Pharmacotherapy Services and within established organizational and departmental policies and procedures, the Clinical Pharmacist is responsible for providing pharmaceutical care in a defined practice area(s). Primary, essential job functions include ensuring rational drug therapy, serving as a clinical role model for staff, providing education and training to healthcare professionals. A commitment to patient safety, customer satisfaction, efficient/effective use of resources, teamwork, innovation, and performance improvement is required. 1.Regularly provides clinical pharmacotherapy services via direct patient care in ambulatory clinics. 2. Monitors drug therapy using implicit and explicit criteria to ensure safety and effectiveness. 3. Provides medication management including appropriate monitoring, drug selection, dosing/titration within the scope of collaborative practice agreement, clinical protocols or standing orders. 4. Performs formal and informal medication use consults with appropriate documentation. Recognized by colleagues as a medication use expert in specialty area(s). 5.Participates in the on-call program; appropriately documents and communicates outcomes as needed. 6. Provides direct, effective, and individualized education and counseling to patients and/or family members/caregivers, as appropriate. 7. Demonstrates and documents value to healthcare team through pharmaceutical care contributions/interventions. 8. Promotes medication safety among colleagues and on behalf of patients. Reports safety concerns through risk management reporting system. 9. Participates in development and/or refinement of clinical pharmacy programs. Assesses clinical and financial impact of efforts on an ongoing basis. 10. Evaluates medication use (i.e., formal usage evaluations); effects measurable improvements in medication use process. 11. Assists in development of protocols, guidelines and standing orders.
Qualifications/Requirements:
Education/Training: 1.Graduate of an accredited college/university with B.S. in Pharmacy or Pharm.D. degree is required. 2. Board Certification through Board of Pharmaceutical Specialties (BPS) is required within 2 years of employment or satisfying BPS eligibility requirements, whichever is longer. 3. Completion of an ASHP-accredited pharmacy residency is desirable.
Licensure: Must have a current license in good standing from the Massachusetts Board of Registration in Pharmacy or be eligible for licensure in MA. In lieu of an active MA license, candidates must attain MA licensure prior to completing 1000 hours of work at the Cambridge Health Alliance (CHA). Failure to attain MA licensure within this time period will result in the discontinuation of employment at CHA.
Work Experience: Minimum experience or equivalent outlined below with demonstrated clinical competency required. • Three (3) years' experience as a pharmacist, or • Completion of a PGY1 residency Credentialing: Pharmacists will participate in the credentialing and privileging process (Policy Number A-PHA-0006) as directed by their supervisor to establish membership on the Medical Staff as an Allied Health Professional.
