Psychiatric Case Manager - Lowell, MA

Amergis Healthcare Staffing is partnered with a respected facility in Lowell, MA, currently seeking Case Managers. The Case Manager coordinates referrals, service planning, and client documentation for designated caseload(s).

Position Details:

• Location: Lowell, MA

• Duration: 13 weeks to start, with the likelihood of extension

• Setting: Psychiatric/Behavioral

• Schedule: Monday - Friday, 8am to 4pm, with a standing 12pm-8pm shift. Includes rotating weekends

• Salary: $28/hr

Position Description:

• Provide case management services as part of a multidisciplinary team

• Develop treatment plans with consumers

• Act as a primary case manager to consumers in accordance with Eliot's policies and PACT standards

• Participate in completing assessments as assigned by the Team Leader or Lead Clinician in a timely manner

• Assess clients' functions, strengths, and needs

• Develop therapeutic connections with each client and provide clinical interventions

• Provide problem-solving supports and interventions to clients

• Caseload is 5-8 clients

Qualifications:

• Valid driver's license

• Reliable vehicle, as you will be asked to take clients out into the community

• 1+ years of prior experience working with adults having severe and persistent mental illness
