Population Health Assistant

The Population Health Assistant shall be responsible for:
- Making outreach to patients, scheduling appointments, and engaging with clinical staff to ensure appropriate care is provided for our high risk behavioral health patients
- Providing administrative support for assigned project teams, including development of educational tools and training materials for quality improvement projects and data management related to assigned behavioral health patients
- Preparing monthly or quarterly reports on behavioral health population activities, progress, status, or other special reports, as assigned, to present to the clinical team.
- Drafting written communication and promotional literature as part of the behavioral health management outreach efforts.
- Interacting with and maintaining liaison with staff, faculty, and outside/community agencies in facilitating program objectives.
- Providing administrative support for assigned project teams, including development of educational tools and training materials for quality improvement projects and data management related to assigned population.
- Providing full project coordination, including scheduling patients, meetings, agenda setting, note taking, and action item completion tracking
- Provides direct administrative support in obtaining documentation and other materials for patient care.
- Initiates internal communication regarding discharge and follow-up status of high-risk patients discharging from other hospitals/other BCH departments.
- Collaborates with Primary Care QI team to work on quality improvement initiatives
To qualify, you must have:
- High School Diploma/GED. Bachelor's degree may substitute for one year of experience
- 1 year experience in customer service or clinical research assistant work.
- Strong verbal and written communication skills and active listening skills. Communicates clearly and appropriately.
- Basic computer navigation and keyboarding skills, including ability to multi-task and navigate between multiple systems.
- Basic skill in collecting and analyzing data.
- Strong and accurate data entry.
- Ability to work with confidential information in a professional manner.
- Basic knowledge of Microsoft Office Suite.
