Our Certified Community Behavioral Health Center (CCBHC) located in Whitman is looking for a Licensed Practical Nurse (LPN) to provide direct care services including comprehensive and integrated client-centered care using evidence-based practices both in person and via Telehealth. The LPN will provide brief medical screenings and collaborate with other CCBHC team members, including psychiatric nurse practitioner, substance use clinician, peer recovery coach, and case manager.

Key Responsibilities:

· Conduct medical screenings and direct care services for Certified Community Behavioral Health Clinic (CCBHC) persons served in collaboration with CCBHC providers and the person served Primary Care Provider

· Provide access to comprehensive and integrated client-centered care using evidence-based practice and assessment for the community and target populations

· Interface with key partners involved in the client’s care including other CBHC team members, behavioral health professionals, primary care providers, and social services

· Work as part of a team providing innovative approaches to behavioral and population health management, engaging clients, families and providers in care planning and coordination, decreasing fragmentation of care for the client, to create a replicable

model for increasing access and ensuring care is provided in the best and least acute setting for the client

Qualifications:

Required Education, Skills, and Qualifications:

· Bachelor’s degree in nursing (B.S.N.) from a four-year college or university preferred or Licensed Practical Nurse

· Current Massachusetts Registered Nursing license or Licensed Practical Nurse

· Preferred behavioral health and substance use disorder treatment experience

· Relevant experience and knowledge in addiction recovery, harm reduction, and/or medication-assisted treatment (MAT) preferred

· Must demonstrate sensitivity to cultural diversity, excellent communication, time-management, organizational and interpersonal skills

· Preference given to candidates with fluency in languages prevalent in Southeastern MA

Travel: Up to 30 percent
