Job Title: LPN/LVN – Long Term Care

Location: Falmouth, MA

Duration: 13 Weeks

Hours: 40 Hours/Week

Shift: Evening Shift – 3:00 PM to 11:30 PM

Schedule: Every Other Weekend & Rotating Holidays Required

Compensation:

  • Travel Pay: $48–$51/hour ($1,920 – $2,040/week) – based on experience
  • Local Pay: $40–$45/hour ($1,600 – $1,800/week) – based on experience

Position Overview:

We are seeking a compassionate and experienced LPN/LVN to provide quality nursing care in a Long-Term Care (LTC) setting. The ideal candidate will supervise nursing assistants, support resident care plans, and ensure compliance with healthcare standards while maintaining a high level of patient-centered care.

Responsibilities:

  • Provide direct nursing care to residents in accordance with care plans and physician orders
  • Supervise and direct day-to-day activities of nursing assistants
  • Coordinate shift assignments, resident services, and care activities
  • Complete and maintain resident admission, transfer, and discharge documentation
  • Receive and transcribe physician orders accurately
  • Update resident charts, medication records, and treatment plans as required
  • Document nursing notes thoroughly and accurately
  • Monitor resident conditions and communicate changes to physicians and healthcare teams
  • Ensure compliance with federal, state, and facility regulations and standards
  • Promote a safe, compassionate, and supportive care environment

Qualifications & Requirements:

  • Graduate of an accredited Nursing Program or approved LPN program
  • Minimum 1 year of Long-Term Care (LTC) experience required
  • Active, unrestricted Massachusetts LPN License required
  • Current BLS / CPR / AED Certification required
  • Strong communication and organizational skills
  • Ability to read, write, speak, and understand English effectively
  • Knowledge and skills necessary to provide age-appropriate resident care
