Responsibilities:

• Provide skilled nursing care and assessments under the plan of care

• Administer medications and treatments as prescribed

• Monitor and document patient condition and report changes to physician

• Educate clients and family members on disease management, medication, and care needs

• Collaborate with caregivers, care managers, and healthcare professionals

• Ensure proper infection control practices are followed

• Maintain accurate and timely clinical documentation

Qualifications:

• Current MA LPN or RN license in good standing

• Minimum of 1 year clinical nursing experience (home care experience preferred)

• Current CPR certification

• Reliable transportation and valid driver’s license

• Excellent communication and interpersonal skills

• Must have knowledge about Medicare and Medicaid requirements and documentation

• Supervisory experience is preferred , especially in a home care or community setting

• Ability to work independently and manage time effectively

• Compassionate, patient, and professional demeanor
