Responsibilities

Elwyn seeks a Team Leader who will be responsible for Clinical and Administrative oversite of ACCS Operations, inclusive of supervision of ACCS Direct Care Staff. Ensures that appropriate services and supports are offered to individuals being served within the ACCS Program, based on the guiding principles of the PRISMModel®. Monitors compliance and adherence to FHR Policy & Procedures, CARF, and DMH ACCS Contract Requirements.

Salary: up to $85,000 annually, $5,000 Sign-on Bonus!

ESSENTIAL FUNCTIONS:

LEADERSHIP AND TEAMWORK

  • Manages staff and program in a PRISMModel® based manner.
  • Oversees the coordination of care with MA Behavioral Health Community Partners, One Care and DMH Case Managers to facility optimum service delivery.
  • Provides one-on-one structured supervision to staff on a regular basis.
  • Conducts regular staff meetings, minimum on a monthly basis.
  • Completes 90-day and annual performance appraisal and submits for approval within established timeline.
  • Assists in executing strategic and plan.
  • Develops a strong committed team.
  • Recruiting individuals who are best suited to perform specific jobs within the organization.
  • Identifies staff's career goals and provides assignments to develop skills in the identified area.

OPERATIONS MANAGEMENT

  • Ensures program operations are in compliance with all Agency, ACCS, CARF and PRISMModel® Standards.
  • Reviews individual records (paper and electronic) of staff to ensure that IAPs, progress notes, and all other required documentation is completed in an accurate and timely manner. Monitors the timely and accurate input of data by ACCS Workforce Members.
  • Participates in annual strategic planning and facilitates meeting identified clinical and balanced scorecard measures.
  • Participates on the Quality Improvement and Regional Leaderships teams
  • Analyzes operational effectiveness/workplace challenges, and initiates process improvement measures to promote maximum efficiencies.
  • Assist in ensuring facilities management is in compliance with all OSHA regulations.
  • Demonstrates all actions are carried out in a safe manner rendering a safe environment for all persons, visitors, and employees.
  • Ensures recommended staff trainings are attended; information is transferred and applied in daily activities.
  • Ensures record-keeping and practice, including clinical records management, is clear, timely, and in compliance with agency, contract, regulatory agencies.
  • Completes all internal and external paperwork as necessary, including but not limited to: HR, QM, and Finance.
  • Performs other duties as assigned.

FINANCIAL MANAGEMENT

  • Assists Regional Director in the development and monitoring of budgets.

EXTERNAL/INTERNAL REPRESENTATION AND COMMUNICATION

  • Serves as FHR’s local representative with the funding source.
  • Ensures that funding source requirements are met with the contracting process, licensing, Rehabilitation Option, Human Rights, etc.
  • Assists management in working effectively with the funding source negotiating referrals, and with other local communication.
  • Serves as FHR’s local representative with other community agencies and groups.
  • Ensures effective communication between programs and the Corporate Office Departments.
  • Serves as a role model to staff presenting a professional, ethical, and positive image, as well as communicating the agency's corporate culture and values as stated in its mission statement.
  • Participates in agency-wide projects and committees.

QUALIFICATIONS, EDUCATION AND EXPERIENCE:

  • Licensed Practitioner of the Healing Arts required.
  • Knowledge of Diagnostic & Statistical Manual (DSM) of Mental Disorders and its clinical application.
  • Knowledge and practice of well-established, evidence-based clinical intervention methods and their appropriate applications (e.g., Motivational Interviewing, CBT, Trauma-Informed Care, WRAP, DBT).
  • Multilingual candidates are preferred.
  • Preference for candidates who meet the cultural and linguistic needs of the geographic area.
  • Ability to identify the information needed to clarify a situation.
  • Skill in seeking information from appropriate sources.
  • Abiliry to use skillful questioning to draw out information when others are reluctant to disclose it.
  • Openness to different and new ways of doing things.
  • Ability and desire to work cooperatively with others on a team.
  • Ability to demonstrate interest, skill, and success in getting groups to learn to work together.
  • Ability to express oneself clearly in conversations and interactions with others.
  • Practices attentive and active listening.

  • Obtain MAP certification within six months of hire.
