Clinical Pharmacist, Operations (Hybrid)
- Provides pharmacy information by answering questions and requests of health care professionals and counseling patients on drug therapies and durable medical equipment
- Develops clinic staff’s pharmacy knowledge by participating in clinical programs and training pharmacy staff, students, interns, residents, and health care professionals
- Performs medication reconciliation and comprehensive medication review for clinic patients
- Identifies patient specific medication therapy problems and documents findings in the EHR
- Identifies patient specific barriers to adherence and develops plan to improve medication-taking behavior (ex. prepackaged medication packs, delivery, med synchronization, coordination with inhouse pharmacy)
- Works to top of license including collaborative practice agreements when necessary
- Complies with state and federal drug laws and regulations
- Maintains records for controlled substances and removes outdated and damaged drugs from the pharmacy inventory
- Maintains separate 340B drug inventory as appropriate
- Supervises the work results of support personnel
- Maintains current registrations
- Protects patients and technicians by adhering to infection-control protocols
- Maintains safe and clean working environment by complying with procedures, rules, and regulations
- Contributes to team effort by accomplishing related results as needed
- Other duties and responsibilities as assigned and necessary to successfully oversee Pharmacy Services
- Light work: Exerting up to 20 pounds of force frequently to move objects
- Ensure safe prescribing and dispensing of medication
- Knowledge of FDA, BOP and all applicable regulations
- Ability to manage processes and develop implementation strategies
- Familiarity with adherence packaging and home delivery
- Previous experience in both clinical pharmacy and pharmacy operations
- Strong analytical and organizational skills including ability to analyze data to identify trends and communicate them through effective reporting methods
- Strong attention to detail
- Strong clinical acumen and a commitment to clinical excellence
- Demonstrated success in working as part of a multi-disciplinary team
- Comfort working in a matrixed work environment and balancing priorities
- Ability to work independently in a professional work environment with limited supervision
- Excellent written and verbal communication skills
- Familiarity with 340B program
- Familiarity with the MassHealth ACO program
- Familiarity with Federally Qualified Health Centers
- Experience with anti-racism activities, and/or lived experience with racism is highly preferred
- Graduate degree from an accredited pharmacy college recognized by the American Council of Pharmaceutical Education (ACPE)
- MTM certificate or willingness to complete
- MCSR or willingness to obtain if CPA with prescribing privileges
- Valid state pharmacist license
- CPR or CPR/AED certificate
- Bachelor of pharmacy or Doctor of Pharmacy degree
