Hospice RN Clinical Liaison
RN Clinical Hospice Liaison - Boston / South Shore, MA
Territory: Boston and South Shore
Current RN license required.
Hospice RN liaison, hospice clinical liaison, hospice admissions, and/or home health clinical liaison experience strongly preferred.
Local market knowledge, job stability, established referral relationships and proven performance are must-haves.
Target Candidate
· Experienced RN hospice clinical liaison or hospice/home health professional with established referral-source relationships in the Boston and South Shore market.
· Must be comfortable developing and maintaining relationships with hospitals, physicians, discharge planners, case managers, skilled nursing facilities, senior living communities and other healthcare referral sources.
· Looking for a field-based RN liaison who understands hospice eligibility, patient/family education, referral development, census growth and consistent account management.
Responsibilities
· Develop, expand and maintain strong referral-source relationships across the assigned market.
· Represent hospice services professionally with referral partners, patients, families and community partners.
· Use clinical knowledge to educate referral sources, patients and families on hospice care, eligibility, services and transition planning.
· Identify new referral opportunities and strengthen existing accounts through consistent outside field activity.
· Work closely with internal clinical and operations teams to support appropriate referrals and smooth transitions into care.
Qualifications
· Current, unencumbered Registered Nurse license required.
· Hospice and/or Home Health clinical liaison, hospice admissions, healthcare sales, marketing, business development or referral-development experience strongly preferred.
· Minimum one year of healthcare sales, marketing or liaison experience required; two or more years preferred.
· Hospice and/or palliative care marketing or liaison experience strongly preferred.
· Established Boston / South Shore hospital, facility and referral-source contacts strongly preferred.
· Local market knowledge, job stability and proven, verifiable performance are must-haves.
· Ability to establish and expand relationships with diverse healthcare referral sources.
· Familiarity with CRM/contact management software preferred.
· Strong problem-solving skills and ability to work independently in the field.
· Reliable transportation, valid driver’s license and current auto insurance required.
Experience
· Minimum: One or more years of healthcare sales, marketing, liaison or referral-development experience required; two or more years in hospice/home health strongly preferred.
