The Nurse Director, Case Management provides leadership to case managers, social workers, utilization review nurses, and transition care coordinators. The position works closely with physician advisors, finance, quality, payor relations, and population health teams to drive strategy, optimize processes, and achieve measurable outcomes.

The Nurse Director Case Management oversees a vibrant, fast-paced department spanning inpatient units, observation, emergency services, and ambulatory care coordination. The team includes RNs, LPNs, social workers, and non-clinical support staff, collaborating in a highly professional, high-energy environment. This unit thrives on teamwork, innovation, and problem-solving to ensure safe, efficient, and patient-centered care transitions across the continuum.

The Nurse Director Case Management will elevate clinical practice through education, competency development, mentorship, and change leadership focused on safe transitions and evidence-based utilization review.
