RN - Wellness Nurse

• Provide nursing care to participants and carry out physician orders
• Assess medical needs, safety risks, and overall well-being
• Develop and update care plans in compliance with MassHealth and DPH regulations
• Supervise and support nursing staff in daily care activities
• Help maintain a safe, structured, and engaging environment for participants
• Ensure compliance with HIPAA, patient safety, and care standards

• ​ Active, unencumbered RN license in Massachusetts
• CPR & First Aid certification
• At least 1 year of nursing experience (geriatric or related setting preferred)
• Strong assessment, critical thinking, and communication skills
• Ability to lead and work collaboratively with a care team
• Comfort using EHR systems and basic computer application
• Physical ability to stand, sit, walk, bend, and squat for extended periods.
• Capable of frequently lifting, carrying, pushing, or pulling objects up to 20 pounds unassisted, and up to 50 pounds or more with assistance as needed.
• Strong interpersonal skills, with the ability to interact professionally and effectively with residents, families, visitors, government personnel, the public, and colleagues.
• Proficient in reading, writing, and following instructions in English.
